Lab Permits


Lab Permits are Required for:

  • Contractors
  • Non-EH&S Staff
  • Machinists/Building Manager
  • Data Collectors
  • Facilities Guided Tours

Lab Permits are NOT Required for:

  • Researcher Guided Tours
  • Researchers
  • Students/Faculty

For those who must submit lab permits prior to entering a lab, there are two categories under which they may submit. All submittals will be logged in the Shared Drive.

1. A “Lab Entrance Request” applies to anyone entering a lab that does not intend to perform work or modify the current equipment/lab environment (e.g. does not adjust or modify dampers, fume hoods, electrical circuitry, experiments, ceiling tiles, flooring, walls, etc.). The Lab Entrance Request must be approved by a supervisor before the entrant may enter the lab. All Lab Entrance Requests must be submitted at least a week prior to the date listed. A person acting under FSU Facilities may enter a lab after having submitted the request and having it approved by a supervisor, but, unlike a Lab Work Permit, the Lab Entrance Request does not require a signed consent form from the researcher/lab owner. However, the form will be Cc’d to the researcher, and, should there be any scheduling conflicts or limitations on the researcher’s end, the entrant may be required to change the date they intend to enter the lab space.

If any changes are inadvertently made during time spent in that lab under a Lab Entrance Request, they must be reported immediately. Likewise, any injuries that result during their time in the lab should be reported, as well.

2. “Lab Work Permit”, applies to anyone entering a lab with the intent to change or modify the current equipment/lab environment. All Lab Work Permits must be approved by both a supervisor and the researcher/lab owner prior to the entering the lab space. The scope of work must be fully defined in the Lab Work Permit; any work not included in the initial submittal must be submitted on a separate form for its own approval even if the dates in the lab coincide. All Lab Work Permits must be submitted at least two weeks in advance of the dates listed. If major changes are required, it is recommended that the form is submitted a month in advance. If any changes are inadvertently made during time spent in that lab, they must be reported immediately. Likewise, any injuries that result during their time in the lab should be reported, as well. The form will be sent to the researcher, and, should there be any scheduling conflicts or limitations on the researcher’s end, the entrant may be required to change the date they intend to enter the lab space.

For those who are not required to submit documentation, it is recommended that prior consent from the researcher/lab owner is received before entering.
